I had notice some mapping issue from the titan pack some time ago, I never post that pics before, it's just to show you even pro make not perfect work lol
There are all from Eden, Inc. probably the level designers were a bit to drunk the day they have build this level lol, just take a look :

All that deco room is quick made and really don't make sence (plants are put on metal stuff etc...) :

héhé.....eagleeye on thoses ones....the plants through the grid dont make sence if you compare the holes and the "trees" ....and it look uggly... right!
Well, you will find many errors like that in all retail maps & even very polished custom maps.
I think they believe that most players won't notice those errors so they don't bother to make all things 100% aligned nor logic. It's obvious for us but not for the majority of players.

I noticed this same thing in some of the UT2K4 maps as well, some grass blades floating in mid air in CTF-Maul, etc. I've flown around the map in game a lot during my rework on the map for UT3, i've really noticed the problems i had never thought to look for before, i think some game development companies need some guys like us working for them. ;)
