the epic'scode of the ladder have a bug it's true, anyway you can use it only in one direction. So you can make a test map to see the direction who work, and after, rotate your entire map in the good direction according to the ladder.
Yeah, I've read the ladder volume only work in the "X" direction. "Y" & "Z" directions doesn't work.
But I don't have the directional arrow to orient the ladder volume in the X direction in the first place !
The ladder SM is also inside the Ladder volume as it's supposed to be. I can see a big green square when I select the ladder SM but I don't understand why it is there in the first place ?

-first, you have to orient your entire map according to the place you want your ladder, so if the wall you want have a ladder has it face oriented to Y, rotate all your map so your wall face is oriented to +X.
-Now, create a volume with the red brush who fit the place you want your ladder and make a right clic on the icone volume tool and select "ladderVolume"
- I rotate the map - Create the ladder volume & make sure "X" of the builder brush is north oriented. The volume is facing the BSP wall behind the foliage - Put the SM inside (you can see the ladder in the geometry) - Rebuild all & nothing happen, I just collide instead of climbing the SM
Since it's not working I also did other actions :
In the ladder volume, I add a positive value in the "Z" field. In the SM, I attach it to the ladder volume
Still no result :(

Ok, you don't need to set something on the ladder volume.
As I can see on your pics, the ladder volume is inclinated, it's the trouble.
-> Your ladder volume has to be parallelto the wall. After rebuild the path, you have to see the 2 small gray ladders icone in each bottom of the volume, they are the entrance/exit of the ladder (on your pics they are both on the middle of the volume, it cannot work)
And don't forget to remove collision on your SM ladder.
After I post the pic, I did put the back of the volume straight to the wall. It wasn't working.
I still didn't make the front straight so the inclinaison is maybe the cause indeed.
The thing is that I don't want the SM to be vertical, I want to place it in 30 / 45 degree so that is why I made the volume as the same shape as the SM in the beginning.
